MANKATO EAST BOOSTER CLUB
1. Please complete the attached Fund Request Form at least one month in advance of the point in time at which you will need the funds.
2. Mail or deliver this Request to:

Mankato, MN 56001
3. Once this Request is submitted, be certain to have a representative at the next scheduled meeting of the Mankato East Booster Club to answer any questions and further explain the request.
4. Final approval of the requests should be approved by the Booster Club and recorded in the minutes of the monthly meeting.
5. The approved requests should be signed by a Booster Club officer, other than the officer issuing the check.
6. A record of all requests and responses to such requests should be maintained in a separate file.
